Get A Copy Of Birth Certificate In New Mexico

The New Mexico Department of Health issues birth certificates for newborns.


Birth certificates are issued to newborns in the State of New Mexico once their parents send the application for a birth certificate to the state Health Department. Over the years, birth certificates have a way of becoming lost or misplaced, which is why the New Mexico Department of Health (DOH) makes it easy to obtain a new copy should you require one. The New Mexico DOH offers several ways you can obtain a new copy of your birth certificate.


Instructions


In Person


1. Visit one of the New Mexico Bureau of Vital Records and Health Statistics offices to apply for a birth certificate in person. Fill out the application provided and return it to the clerk. Office hours at the Santa Fe offices are Monday through Friday, 8:30 a.m. to 4 p.m. and the Albuquerque office from 9 a.m. to 4 p.m.


2. Provide the office clerk with a copy of your photo identification. Copies of birth certificates are only issued if it is your certificate or the certificate of someone in your immediate family.


3. Pay for the copy of your birth certificate. As of 2010, the fee per certificate is $10, payable by cash, check or credit card. Your birth certificate copy is given to you while you wait.


By Mail


4. Download a copy of the birth certificate application or compose a letter requesting a copy of the certificate (see Resources). Include the name listed on the certificate, date of birth, location of birth, parents' names, your relationship to the person listed on the certificate, the reason you need a copy of the certificate, your signature and your mailing address and contact information.


5. Write a check or money order payable to New Mexico Vital Records in the amount of $10. Include a self-addressed, stamped envelope.


6. Mail your request, envelope and payment to:

Your request is processed and mailed within four weeks of receipt.


Online


7. Order your birth certificate online using the New Mexico DOH's third-party site, VitalChek (see Resources).


8. Enter the required information as you are asked for it and choose a shipping method.


9. Provide your shipping address, contact information and your credit card number. Your certificate is mailed to you based on the shipping method you paid for.
